Parasuicide by self-injection of an organophosphate insecticide

Parasuicide by ingestion of organophosphate (OP) insecticides is common in Sri Lanka, but the use of the parateral route to self administer the poison is extremely rare. We report a patient who deliberately injected herself intramuscularly with an OP compound with suicidal intent.

Organophosphates are irreversible acetylcholinesterase inhibitors. Intoxication with these compounds is a global health concern. Acute manifestations include muscarinic, nicotinic and central nervous system signs and symptoms. Diagnosis requires a high clinical index of suspicion.

AbstractAn enzyme preparation that could detoxify parathion and eight other organophosphate pesticides was covalently bound to either porous glass or porous silica beads. This immobilized‐enzyme system was examined for its use in detoxification of pesticides in production wastewaters.

Administration of apparently nontoxic doses of organophosphorus compounds can greatly alter the toxicity of other compounds. These might include other organophosphates, as well as other drug or nondrug xenobiotics. Mechanisms of toxicologic interactions are discussed.
