Osteoid Osteoma of the Hamate

Uncommonly affecting the carpus, osteoid osteoma, a benign bone lesion, will involve the hand in 10% of cases. With negative initial radiographs and persistent tenderness in a 15-year-old boy after a fall, a wrist magnetic resonance imaging revealed extensive hamate edema without a fracture line. He was casted for a presumed hamate fracture.

Osteoid osteoma of the hand

A review of all cases of osteoid osteoma of the hand seen by four hand surgeons over the last 10 years was performed. Seven cases were documented. Average follow-up was 28.3 months. Average age of the patients was 21.1 years. Five men and two women participated. Six lesions were in the right and one in the left upper extremity.

Prostaglandins in osteoid osteoma

Osteoid osteoma is a tumour of bone characterised by pain which is relieved by aspirin and nonsteroidal anti-inflammatory drugs. Very high levels of prostaglandins have been found in the lesion. The clinical records and roentgenograms of fifty-four patients had the typical features of osteoid-osteoma, but no histological evidence of a nidus was found at initial surgery. The symptoms of thirty-one patients were relieved by initial operation and those of twenty-three were not.

Osteoid osteoma of the acetabulum

Osteoid osteoma of the acetabulum can be expected to cause nonspecific symptoms of hip inflammation. In a sixteen year old girl, investigation by routine radiographs and a bone scan suggested a focus of inflammation with a nidus and sclerosis of the acetabulum and overgrowth of the head and neck of the femur.

GIANT OSTEOID OSTEOMA

We have discussed the pathological and clinical features in eleven examples of a distinctive type of benign tumor of bone. These tumors are characterized by a prominent network of osteoid trabeculae showing varying degrees of calcification and a vascular stroma which fairly frequently contains benign giant cells and little fibrous tissue.

Osteoid osteoma of the axis

A 15-year-old boy suffering from osteoid osteoma of the right lamina of the axis presented with torticollis and local pain without neurological deficits. The tumour was treated by en-bloc resection and laminoplasty with autogenous iliac crest. The follow-up at 14 months showed complete pain relief and a perfect incorporation of the transplanted bone.
