Neurosarcoidosis presenting overt myopathy.
症例は64歳,男性.視力低下に続き,手足のしびれ出現.両虹彩毛様体炎,嚥下障害,近位筋優位の筋力低下あり,血清リゾチーム上昇,縦隔リンパ節腫脹,髄液細胞数増加,針筋電図で筋原性変化,多発神経炎型末梢神経障害を認めた.腓腹神経生検で類上皮細胞を形成成分とする肉芽腫を,上腕二頭筋生検で小径角化線維の散在を認め,ミオパチーを合併した神経サルコイドーシスと診断.筋はサルコイドーシスの好発部位だが,症状を呈することは極めて稀で,貴重な症例であった.
