AbstractA nine‐year‐old child presented with congenital insensitivity to pain and anhidrosis. Quantitative studies and electron microscopy of the cutaneous branch of the radial nerve revealed almost complete absence of small myelinated and unmyelinated fibers and a disproportionate number of nerve fibers with a diameter of 6–10 μm.

Congenital insensitivity to pain with anhidrosis is an autosomal recessive disorder included in a group of rare diseases termed as hereditary sensory-motor neuropathies. The diagnosis is made usually in early childhood period as most of the children present with recurrent unexplained hyperpyrexia.

SUMMARYContrary to repeated statements in the literature, most mental defectives respond appropriately to painful stimuli. An exceptional case of congenital insensitivity to pain in a mentally backward boy is described. The defect resulted in multiple burns and sores with septic complications and urinary infection, leading to complete disability.
