Use of an extended superiorly based posterior wall pharyngeal flap for the immediate reconstruction of the soft palate in three patients undergoing commando excision of oropharyngeal neoplasms recurrent after radiotherapy is described. The technique appears to be simple, fast, safe, and effective.
