Cancer of the pancreas is the tenth most frequent cancer in Europe, accounting for some 3% of cancer in both sex. Smoking has been clearly established as a major risk factor affecting the carcinogenesis of pancreatic carcinoma. Chronic pancreatitis is frequently observed in men drinking approximately 2 g·kg<sup>-1</sup>day<sup>-1</sup> during a mean of 17 years. The diet of these patients is abnormally rich in fat and protein. Giving alcohol and a similar diet to dogs and rats, it has been possible to reproduce chronic alcoholic pancreatitis.

Severe acute pancreatitis with pancreatic necrosis usually precedes a pancreatic abscess. The clinical features are not always dramatic, but when malaise, fever and leucocytosis persist after an attack of severe acute pancreatitis, this complication should be suspected. Surgical drainage of the abscess provides the only hope of survival.

High mortality and overall discouraging results of pancreaticoduodenectomy misled some physicians to believe that surgery is ineffective in management of pancreatic carcinoma.
