The Japanese Journal of Gastroenterological Surgery, 1990 腸間膜脂肪織炎 (mesenteric panniculitis) は, 腸間膜脂肪織に生ずるきわめてまれな非特異性の炎症疾患である.われわれは, 横行結腸間膜に嚢胞状に発生し, 感染, 穿破して急性腹膜炎で発症した脂肪織炎の1例を経験した.症例は, 11歳の男児で, 発熱, 腹痛を主訴に外来受診した.腹部全体に圧痛と筋性防御があり, 白血球数の上昇を認めた.腹部超音波で, 上腹部に嚢胞状の腫瘤を認めた.急性腹膜炎として緊急手術を施行, 横行結腸間膜に嚢胞状で一部穿破した腫瘤を認めた.嚢胞切除と腹腔ドレナージ術を施行した.切除標本の病理組織学検査にて, 腸間膜脂肪織炎と診断された.本症例は, 急性腹膜炎にて発症した点, また嚢胞状を呈していた点など今まで報告のないものであった ...

The Japanese Journal of Gastroenterological Surgery, 2000 症例は48歳の男性. 発熱, 腹痛を主訴に来院, 腹部全体に反跳痛, 筋性防御が認められ, 汎発性腹膜炎の診断にて開腹した. 手術所見では漿液性腹水少量, 小腸間膜の肥厚, 発赤を認めるのみであった. 腸間膜脂肪織炎を疑い一部生検を施行し, 閉腹した. 術後39℃の発熱が8日目まで持続したが, γ-globulin投与したところ平熱化し, 炎症は鎮静化した. 病理組織所見では, 変性脂肪細胞, 炎症細胞浸潤, 微小膿瘍が認められ, 脂肪織炎と診断した. 腹水細菌培養は陰性であった.腸間膜脂肪織炎は原因不明の比較的稀な疾患であり, その臨床像は多彩なため診断は困難で確立した治療法はない. 今回, 我々はγ-globulin投与が奏効したと思われる1例を経験したので文献的考察を加え報告する.
