ストーマ旁ヘルニアは人工肛門造設術後の比較的頻度の高い合併症である.しかし,外科治療法は確立されておらず,その再発率の高さや侵襲性が問題となっている.近年再発率の低減を目指したメッシュを用いた修復術の報告が散見されている.しかし術式は統一されておらず感染や腸管との瘻孔形成などの合併症も報告されている.今回,我々は非開腹下にヘルニア門を縫縮後Marlex Mesh®をonlay法(筋膜上留置)で挿入し,さらに縫縮部減張のため外腹斜筋腱膜切開を加えた修復術を行い,低侵襲で良好な経過を得られた.当院で行った同修復術3例とストーマ旁ヘルニア修復術に関する本邦報告例36例を併せて検討し報告する.
