Parathyroid carcinoma is a rare form of endocrine malignancy accounting for only a small minority of cancer cases. Due to the rarity of this cancer, there are no generalized guidelines for its management; however, surgery remains to be the mainstay therapy.

We report on eight cases of parathyroid carcinoma seen at the Bowman Gray School of Medicine, Winston-Salem, NC, since 1969. Diagnosis of a parathyroid disorder was made on the basis of elevated serum calcium levels and associated disorders such as renal calculi, peptic ulcer disease, pancreatitis, and demineralization of bone.

The histologic diagnosis of carcinoma of the parathyroid is difficult to make, and invasion of contiguous structures or metastasis must be present before the diagnosis can be established. The serum calcium level is usually high, and the tumor is palpable in approximately one half of the patients.
