Near-total laryngectomy (NTL) provides a func-tional alternative to total laryngectomy (TL) in T3-T4laryngeal and hypopharyngeal cancer. It was initiallyadvocated in extended glottic carcinoma and in hypo-pharyngeal carcinoma. More recently, the utility ofPearson’s NTL for supraglottic, pharyngeal, and base oftongue cancer has been reported.

Clinical experience of laryngeal cancer gave a relatively better prognosis compared with cancer, in other locations in our field. Histological examinations of laryngeal cancer showed that in most of our cases the tumours had an even, microscopically localized growth. Serial sections in our cases confirmed this observation.

Partial Laryngectomy after Irradiation Failure

Forty-three patients who had partial laryngectomies after radiotherapy were evaluated. Forty-two had failed irradiation for laryngeal carcinoma. Nine had supraglottic procedures, 3 endoscopic, and 31 vertical partial. Twelve (28%) required subsequent completion laryngectomy.
