Patient Ambulations Predict Hospital Readmission
2021 43rd Annual International Conference of the IEEE Engineering in Medicine & Biology Society (EMBC), 2021Improved functional ability and physical activity are strongly associated with a broad range of positive health outcomes including reduced risk of hospital readmission. This study presents an algorithm for detecting ambulations from time-resolved step counts gathered from remote monitoring of patients receiving hospital care in their homes. It examines

Nonelective readmissions of medical patients
Journal of Chronic Diseases, 1985Because it is difficult to predict the course of patients discharged from the hospital, we studied 1646 consecutive patients discharged from the medicine service to the care of the hospital's medical staff. Conditions serious enough to warrant urgent or emergent (nonelective) readmissions developed in 278 (16.9%) within 90 days.

Hospital Readmissions from Patientsā Perspectives
Southern Medical Journal, 2017Healthcare expenditures in the United States have increased exponentially and hospital care accounts for one-third of these costs. Cerebral cavernous malformations (CCMs) are microvascular CNS lesions prone to hemorrhage leading to neurological sequela such as stroke and seizure. A subset of CCM patients have aggressive disease leading to multiple bleeding events, likely resulting multiple hospitalizations. Hospital admission rates are an important metric that has direct financial

Hospital Readmissions in Hospice Patients: Evaluation of Medication-Related Causes for Readmission
American Journal of Hospice and Palliative MedicineĀ®, 2020Context: Hospital readmissions, frequent medication changes, and polypharmacy are common issues for hospice patients. It is important to consider if close monitoring of medications by pharmacists could help these patients avoid hospital readmissions. Objective: The objectives of this study were to determine the incidence and types of medication-related
