Hospital readmissions are a frequent challenge. Speculation exists that rates of readmission following traumatic injury will be publicly disclosed. The primary aim of this study was to characterize and model 1-year readmission patterns to multiple institutions among patients originally admitted to a single, urban Level I trauma center.

Patient-reported oncology readmission factors.
Journal of Clinical Oncology, 201248 Background: Reducing the risk of readmissions is an important quality goal for hospitals. Little is known of the perspectives of patients on underlying challenges that may lead to readmission. The objective of this study was to elicit perspectives of readmitted oncology patients on barriers to a successful transition from hospital to home. Methods:

Hospital Readmission of Patients with Diabetes
Current Diabetes Reports, 2015Hospital readmission is a high-priority health care quality measure and target for cost reduction. Despite broad interest in readmission, relatively little research has focused on patients with diabetes. The burden of diabetes among hospitalized patients, however, is substantial, growing, and costly, and readmissions contribute a significant portion of

Predicting Hospital Readmission of Elderly Cardiac Patients
Health & Social Work, 1987Sixty hospitalized elderly cardiac patients were tested using the Geriatric Functional Rating Scale (GFRS) and were followed four months postdischarge. Significant relationships were found between GFRS scores and readmission status. These data substantiate the potential usefulness of GFRS as a predictive instrument to identify those high-risk ...

Patient learning center reduces patient readmissions
Patient Education and Counseling, 1991Abstract This study examined the effectiveness of a Learning Center in preparing patients to go home with a right atrial catheter. Two hypotheses related to the intended purpose of the Learning Center were examined—number of learning outcomes achieved would be higher and readmission rate for complications related to the catheter or for reinsertion of

Hospital Readmission Among Long-term Ventilator Patients
Chest, 2001Patients experiencing prolonged periods of in-hospital mechanical ventilation have been described as long-term ventilator (LTV) patients. The purpose of this study was to document the incidence of hospital readmission and to identify risk factors for readmission for LTV patients up to 6 months after hospital discharge. This study was part of a larger ...
