Carcinoid constrictive pericarditis [PDF]

A 78 year old man presented with diarrhoea, anorexia, and progressive lower limb oedema. He was in atrial fibrillation and had a right pleural effusion and ascites. Ultrasound of the abdomen and 24 hour urinary hydroxyindoleacetic acid output indicated metastatic carcinoid syndrome.

Constrictive pericarditis (CP) is a rare clinical entity that can pose diagnostic problems. The diagnosis of CP requires a high degree of clinical suspicion. The gold standard for diagnosis is cardiac catheterization with analysis of intracavitary pressure curves, which are high and, in end diastole, equal in all chambers.

Afatinib Versus Osimertinib for Non‐Small Cell Lung Cancer With Uncommon EGFR Mutations: Real‐World Outcomes

Afatinib and osimertinib showed broadly comparable survival outcomes as first‐line therapy for NSCLC with uncommon EGFR mutations. Treatment may be individualized by mutation subtype, while adding immune checkpoint inhibition to platinum doublet after EGFR‐TKI showed no clear benefit.
