Successful Delayed Elective Separation of Pygopagus Conjoined Twins With Shared Spinal Cord and Dura in a Resource‐Limited Setting: A Case Report

open access: yesClinical Case Reports, Volume 14, Issue 10, October 2026.
ABSTRACT Conjoined twins are a rare consequence of aberrant monozygotic twinning; one of the varieties is pygopagus twins, which are fused at the sacral region and may involve complex neural and dural sharing. We report a case of female pygopagus conjoined twins with shared spinal cord and dural sharing who underwent successful delayed elective ...

How to Avoid Misdiagnosing Primary Spinal Epidural Abscess as Spinal Epidural Hematoma: A Case Report and Literature Review

open access: yesClinical Case Reports, Volume 14, Issue 10, October 2026.
ABSTRACT Spinal epidural abscess (SEA) is an easily misdiagnosed disease. We report a case of primary SEA (PSEA) that was misdiagnosed as spinal epidural hematoma (SEH) and analyze the reasons. The misdiagnosis was confirmed intraoperatively. Comprehensive analysis is crucial for the diagnosis of PSEA and its differential diagnosis from SEH.

Introducing A Stepwise Anatomic Framework for Urogenital Reconstruction Derived From Orofacial Plastic Surgery Principles: A Case Report

open access: yesClinical Case Reports, Volume 14, Issue 10, October 2026.
ABSTRACT We present a novel labial reconstruction technique in a 48‐year‐old woman with extensive bilateral labial defects caused by steatocystoma multiplex. An anatomy‐guided, stepwise approach integrating scalp and orofacial reconstructive principles enabled successful restoration using a rhomboid mucocutaneous flap, minimizing tissue tension while ...

Deep Gluteal Syndrome and Persistent Sciatica in a Patient With Unilateral Piriformis Muscle Hypertrophy: A Case Report

open access: yesClinical Case Reports, Volume 14, Issue 10, October 2026.
ABSTRACT A man of 47 had 6 months of left sciatica without root compression on lumbar imaging. Pelvic CT showed isolated left piriformis hypertrophy, 700 versus 450 mm2. Guided betamethasone injection relieved him within 48 h, sustained at 6 months. Asymmetry and response support, but do not prove, sciatic nerve entrapment.

SGLT2i in heart failure: Cardiorenal mechanisms, clinical evidence and translational perspectives

open access: yesClinical and Translational Discovery, Volume 6, Issue 5, October 2026.
SGLT2 inhibitors improve heart failure outcomes through integrated cardiorenal mechanisms. In the kidney, they reduce glucose and sodium reabsorption, promoting natriuresis, osmotic diuresis and renal protection. In the heart, they enhance myocardial energetics, reduce inflammation, oxidative stress, fibrosis and adverse remodelling, resulting in fewer
