Well‐Defined Poly(HPMAm) Brushes via Surface‐Initiated RAFT Polymerization; A Mixed‐Chain Transfer Agent (CTA) Approach

open access: yesAdvanced Materials Interfaces, EarlyView.
A simple method relying on surface‐initiated RAFT polymerization is presented using a mixed‐chain transfer agent (mixed‐CTA) approach to grow well‐defined poly(HPMAm) brushes with precise control, achieving >70 nm thickness in 4 h at moderate temperature, 50 °C. Abstract Over three decades, efforts to prevent non‐specific protein adsorption on surfaces

Planning for Pharmaceutical Care

American Journal of Health-System Pharmacy, 1993
The process for planning a pharmaceutical care practice model at the University of California-San Diego Medical Center (UCSDMC) is described. Pharmaceutical care is viewed as a health care need, analogous to medical care or dental care. Provision of such care requires that pharmacists have a generalist, rather than specialist, orientation and be held ...

Pharmaceutical care

Pharmacy World and Science, 1996
Drug-treatment failures can be prevented by applying a Pharmaceutical Care system. Therapeutic outcome monitoring is such a system, which can be applied to the (drug) treatment of several diseases like asthma, diabetes and cardiovascular diseases. Pharmaceutical Care is an outcome oriented, cooperative, systematic approach to providing drug therapy ...

Pharmaceutical Care in Pediatrics

2018
When providing pharmaceutical care to the pediatric population, pharmacists need to take extra care, and be vigilant to try to prevent some of the common drug-related problems that have previously been reported too commonly for this cohort. Through the medication review process (see Chap.

Documenting Pharmaceutical Care

2018
Documentation of pharmaceutical care has been advocated for close to 30 years for a number of reasons, not least to enable patient follow-up of managed or resolved drug-related problems (DRPs). The first pharmaceutical care documentation was performed in the US, followed by the Netherlands and Sweden, and has also been performed in Canada, in ...

Pharmaceutical Care in the Aged

2018
Prescribing for older people is complex particularly in the presence of multi-morbidity. Ensuring the appropriate balance of prescribed medicines requires a consideration of pharmacokinetic and pharmacodynamics parameters, a knowledge of the potential adverse effects as well as the potential for inappropriate prescribing. Adopting a systematic approach
