2020A 17-year-old female is seen in the emergency department with a 2-day history of fevers to 103 °F and a sore throat. Today, she describes her pain to be severe and mostly on the right side. She complains of difficulty opening her mouth and pain with swallowing. On examination, she appears anxious and uncomfortable, speaking softly with a muffled voice.

Oropharyngeal cultures for Chlamydia trachomatis were negative in all women, including 11 women who practiced fellatio and whose partners were known to have nongonococcal urethritis due to C.

Although the group A beta-hemolytic streptococcus is the most important treatable cause of pharyngitis, other organisms, including viruses and groups C and G streptococci, should always be considered in the diagnosis of pharyngeal symptoms. Clinical findings and a rapid strep test aid in making the diagnosis of group A streptococcal pharyngitis.
