Pharyngocutaneous fistula after total laryngectomy
Toukeibu Gan, 2010喉頭全摘術後の咽頭皮膚瘻(PCF,pharyngo cutaneous fistula)発生とその閉鎖に要した期間について比較検討した。対象は当科にて喉頭全摘術を施行した78症例で,内訳は初回治療として喉頭全摘術を施行された症例(PL)が43症例,救済手術症例(SL)が35例であった。全78症例中23例(29.4%)にPCFを認めた。PLに比べてSLではPCF発生が有意に増加した(p < 0.05)。瘻孔閉鎖に要した期間は,PLに比べてSLで有意に長かった(p < 0.05)。全身化学放射線療法併用後の救済手術例に比べて動注化学放射線療法後の症例では長期化する傾向を認めたが,明らかな有意差は認められなかった。PCF発生に関しては放射線照射の影響が強く ...
