We report an extremely rare case of encysted pneumomediastinum with septations following talc pleurodesis for a secondary spontaneous pneumothorax. Respiratory Physicians should be able to diagnose abnormalities seen in the chest x‐ray, and further evaluation should be launched based on this finding. Talc pleurodesis is a recommended treatment modality
