Pneumopericardium in the Neonate
AbstractPneumopericardium occurs when air accumulates in the pericardial sac surrounding the heart and is one of the rarest forms of air leaks in neonates. Because of various advances in neonatal care, including gentler modes of ventilation, surfactant replacement, and antenatal steroids, the incidence of pneumopericardium has decreased.

Pneumopericardium is the least common form of air leak in infants. A tension pneumopericardium is even more infrequent but associated with a very high mortality rate.

Three cases of neonatal pneumopericardium are presented. Detailed ventilatory measurements and chest roentgenographic changes are described for each of the three patients. The literature of neonatal pneumopericardium is reviewed, and based on that review and our own cases, we offer guidelines for managing these patients.

We would like to add a case of pneumopericardium in the newborn to the seven reviewed by Matthieu, et al.1 This case is unusual in that it is only the second reported in which the pneumopericardium cleared spontaneously without aspiration. V. J. was a premature, Negro, female infant born at 26 weeks' gestation to a 14-year-old Gravida I,

Four neonates with pneumopericardium were seen within a 12-month period. All infants received various methods of assisted ventilation. Two episodes were recognized early and prompt pericardial aspiration resulted in survival. One was asymptomatic, recognized by roentgenogram and treated conservatively.

The clinical data and radiographs of 50 neonates with pneumopericardium (PPC) were reviewed. Most of these neonates were premature, low-birth-weight infants with hyaline membrane disease who required ventilatory assistance. The presence of other extraventilatory air prior to (or concurrent with) PPC, the use of cardiopulmonary resuscitation with ...

Postpartum pneumopericardium has been previously reported in only 4 patients, but it may occur more frequently than is currently recognized. Two patients are presented with pneumopericardium and pneumomediastinum with symptoms appearing on the first and second postpartum days. The second stage of labor was prolonged in both patients and associated with

Pneumopericardium is an unusual complication of mediastinal emphysema in infants. It is rarely recognized clinically, yet the symptoms and signs are characteristic, and the prompt relief of the gas tamponade by pericordotomy is lifesaving. Six infants with this condition have been observed in the past 2 years, and all were receiving positive pressure ...

Pneumopericardium in the Newborn
Pediatrics, 1970Pneumopericardium complicating the course of hyaline membrane disease in a newborn, premature infant is reported. The details of other instances of neonatal pneumopericardium although unassociated with hyaline membrane disease are reviewed. The clinical manifestations, similar to those of pneumomediastinum, appear very suddenly and can lead to cardiac ...
