ABSTRACT An 86‐year‐old woman with a tracheostomy developed airway obstruction caused by viscid sputum. Forceful manual ventilation through her obstructed airway precipitated tension pneumothorax, pneumomediastinum, pneumopericardium, pneumoperitoneum, and pneumoretroperitoneum.
