Polyarteritis nodosa (PAN) is regarded rightly as the grandfather of the vasculitides. In this Grand Rounds, the case of a 30-year-old man with a 12-year illness is described. The patient presented with daily fevers, tachycardia, and cutaneous ulcers on his distal extremities. He eventually developed mononeuritis multiplex.

The underlying disease was not recognised and the patient was wrongly thought to have a penile cancer. Treatment with radiotherapy and Bleomycin had no effect and he subsequently died in uraemia.

Magnetic resonance imaging of the lower limbs demonstrated highly increased signal intensity in both soleus muscles and the lateral head of the left gastrocnemius muscle.

[1] describe the radiologic findings of polyarteritis nodosa in the abdomens of seven patients. Their conclusion “vasculitis should be considered when multiple abdominal organs have abnormal radiologic findings” is beyond dispute.
