Too few myocardial infarctions are thrombolysed, and the thrombolytic agent is usually administered too late. This situation can conceivably be improved by educating both physicians and patients, by promoting thrombolysis in all hospitals and by performing thrombolysis before admission.

Survival after drowning depends on a well-functioning chain of survival, starting outside the hospital and continuing in-hospital during the post-resuscitation phase. Prehospital care personnel provide the opportunity to perform rapid rescue from submersion events.

Organisatory and therapeutic characteristics for selected pre-hospital medical treatment systems are described. It is emphasized that all of the links in the therapeutic chain must function optimally to achieve effective treatment of acute disease and injuries. The importance of rapid decision-making by highly qualified medical staff is emphasized.

Abstract Patients begin to die early and unpredictably after myocardial infarction; about two-thirds of the deaths associated with myocardial infarction take place out of hospital, and 50 per cent of the patients who will die do so within two hours of the onset of symptoms, mostly as a result of ventricular fibrillation.
