We evaluated the efficacy of initiating a second trimester medical abortion outside of a health care facility using patient self-administered serial intravaginal misoprostol. Patients scheduled for second trimester medical termination of pregnancy were randomized to an inpatient or outpatient group.

Over three days, the patient’s hemoglobin concentration declined from 134 g/L to 56 g/L. Repeat ultrasound showed a live pregnancy (Figure 1, white arrow) outside the uterus (Figure 1, black arrow). At laparotomy, an extrauterine gestational sac was identified in the right adnexa, extending into the pelvic sidewall (Figure 2).
