A 73 years-old male was referred for catheter ablation of left ventricular summit (LVS) arrhythmias. Typical LVS arrhythmias have a right bundle branch block configuration in lead V1 or a left bundle branch block configuration with early precordial transition (V3 or earlier). The axis is always inferior.
