Long‐Term Stoma‐Free Outcomes After Diverting Stoma Closure Following Radiation‐Based Neoadjuvant Treatment for Rectal Cancer

open access: yesAnnals of Gastroenterological Surgery, EarlyView.

Laparoscopic Versus Open Surgery for Diverticular Disease: A Structured Review Across Clinical Settings and Operative Strategies

open access: yesAnnals of Gastroenterological Surgery, EarlyView.
ABSTRACT Laparoscopic surgery is increasingly used to treat colonic diverticulitis; however, its role in emergency, perforated, high‐risk, and procedure‐specific settings remains uncertain. This structured narrative review evaluated and synthesized comparative evidence by clinical setting, operative strategy, and patient risk profile to provide a ...

Minimally Invasive Versus Open Total and Distal Gastrectomy After Chemotherapy for Gastric Cancer: A Nationwide Observational Study

open access: yesAnnals of Gastroenterological Surgery, EarlyView.
This nationwide observational study used data from the National Clinical Database. Patients who underwent total or distal gastrectomy for gastric cancer after chemotherapy between January 2014 and December 2022 were included. Minimally invasive surgery was associated with lower incidences of overall postoperative complications of Clavien–Dindo grade ...

The Geriatric Nutritional Risk Index Predicts Short‐ and Long‐Term Outcomes in the Oldest‐Old With Colorectal Cancer: A Multi‐Institutional Analysis of 225 Nonagenarians

open access: yesAnnals of Gastroenterological Surgery, EarlyView.
This multi‐institutional study evaluated the Geriatric Nutritional Risk Index (GNRI) in 225 nonagenarians undergoing surgical resection for colorectal cancer. A lower preoperative GNRI was independently associated with a longer hospital stay and poorer overall survival, even after adjustment for confounders using inverse‐probability‐of‐treatment ...

Exploring Symptom Domains of Postoperative Bowel Dysfunction by Integrating LARS and Wexner Scores: A Longitudinal Factor‐Analytic Study

open access: yesAnnals of Gastroenterological Surgery, EarlyView.
Item‐level factor analysis of the LARS and Wexner scores in 320 patients undergoing sphincter‐preserving rectal resection identified three distinct symptom domains: incontinence, urgency/clustering, and gas control. These domains had different clinical determinants: radiotherapy was associated with greater severity of both incontinence and urgency ...

Survival Results by the Prospectively Determined Clinical Staging for Locally Advanced Gastric Cancer: A Follow‐Up Study of JCOG1302A

open access: yesAnnals of Gastroenterological Surgery, EarlyView.
Prospectively determined clinical T and N categories clearly stratified long‐term survival in locally advanced gastric cancer. Patients with cT3N+, cT4aN0, cT4aN+, and cT4b disease showed unfavorable prognosis despite standard treatment. These findings may help optimize patient selection for perioperative treatment strategies.

Barbed Sutures Are Associated With a Lower Incidence of Incisional Hernia Compared With Continuous Non‐Barbed Monofilament Sutures After Minimally Invasive Gastroenterological Surgery

open access: yesAnnals of Gastroenterological Surgery, EarlyView.
Incisional hernia remains a common complication after minimally invasive gastroenterological surgery despite small extraction incisions. In this propensity score‐weighted analysis, barbed sutures significantly reduced the incidence of incisional hernia compared with conventional slowly absorbable monofilament sutures without increasing postoperative ...
