Proactive Safety Requires Proactive Measurement [PDF]
A major challenge for organizations and safety researchers alike is that commonly used measures of safety (e.g., accident/incident rates, safety behavior) support a reactive rather than a proactive approach to safety. We argue that Reason’s (1990) widely-accepted model of the causes of error suggests a more useful place to focus measurement attempts ...

Causality and Proactive Cancellation
2006 Tenth IEEE International Symposium on Distributed Simulation and Real-Time Applications, 2006Optimistic Time Warp simulators should stop the rapid propagation of incorrect events to avoid reaching a catastrophic state (a state where out of order event execution is always a step ahead of its corrective measures). A distributed cancellation mechanism using Total Clocks was proposed earlier to avoid such catastrophic states.

AbstractGrowth in health information systems presents opportunities to enhance postmarketing safety surveillance of medical products. Spontaneous suspected side effect reports provide the foundation, but we need to ‘proactively’ improve their quality and our strategies to seek signals.

Proactive interference and complexity.
Journal of Experimental Psychology: Learning, Memory, and Cognition, 2001C. L. Isaac and A. R. Mayes (1999a, 1999b) compared forgetting rates in amnesic patients and normal participants across a range of memory tasks. Although the results are complex, many of them appear to be replicable and there are several commendable features to the design and analysis.
