ALLERGY TO PROCAINE HYDROCHLORIDE WITH THREE FATALITIES

This report deals with three fatalities encountered by us during the past year, resulting from an immediate reaction to procaine hydrochloride. Several other severe but nonfatal reactions were seen by us during the same interval but are not included in this paper.

Attempts to control the pain of joint disease and its associated syndromes have led to the use of procaine hydrochloride. It has been injected intravenously to relieve the pain of chronic arthritis, especially degenerative arthritis. The relief obtained has not been striking or long lasting. Side-reactions have been common.

The ability of procaine hydrochloride (P.HCl) to modulate the effects of cisplatin (DDP) on pluripotent (CFU-S) and committed (CFU-GM) murine hemopoietic stem cells was investigated. DBA/2NCrlBRF1 mice received DDP alone (10 and 16 mg/kg body wt. single i.p. injection) or in combination with P.HCl (40 mg/kg body wt. single i.p. injection).

Stability of procaine hydrochloride in a buffered cardioplegia formulation

The stability of procaine hydrochloride in a buffered cardioplegia solution was studied. The formulation of Ringer's injection with added increments of potassium and magnesium plus procaine hydrochloride was buffered to a pH of 7.3-7.6 with tromethamine.

The local anesthetic procaine hydrochloride (P.HCl) had little effect on the sensitivity of P388 leukemic cells to cisplatin (DDP) in vitro, but the simultaneous administration of DDP and P.HCl (40 mg/kg) to BDF1 mice produced 50% lethal dose (LD50) and 90% lethal dose (LD90) values approximately two times higher than those observed with DDP alone. Procaine amide ("pronestyl") hydrochloride is a drug widely used for the elimination and prevention of ectopic ventricular rhythms. Its oral use at times is maintained for long periods to prevent recurrent premature beats or paroxysmal ventricular tachycardia. Toxic side-effects have been infrequent and usually not serious.
