In Cape Breton Island and southwestern Newfoundland, the Appalachian orogen is extremely narrow; the rocks were strongly deformed and underwent high-grade Barrovian-type metamorphism in Silurian time, and the orogen-parallel Silurian thrusts are dominantly west-vergent, antithetic to the westward subduction.

The thresholds, discomfort levels and dynamic ranges were found to be comparable with a series of ten patients who have subsequently been successfully implanted with the Nucleus 22 channel cochlear implant.

Anatomical variants such as an overhanging facial nerve or promontory can impede access to the footplate during stapedectomy. Drilling away bone from the cochlear promontory may be required. In the case of a floating or depressed footplate, it has been recommended that a "pothole" be drilled in the inferior margin of the oval window.
