Management of PTLD after SOT

No therapies for PTLD after solid organ transplantation are based on data from randomized clinical trials, but available phase II trial data help guide therapeutic decision making. Reduction or cessation of immunosuppression is performed at the time of diagnosis in almost all patients.

T‐cell PTLD presenting as acalculous cholecystitis

Abstract:  We report a five‐yr‐old child, presenting three yr after heart transplant with acalculous cholecystitis. Histology revealed EBV negative T‐cell PTLD. The disease involved the gallbladder, liver, lungs, and mesenteric lymph nodes. He was treated with chemotherapy, went into remission, but relapsed after 11 months and died.

Prognostic Factors for PTLD

Poor prognostic factors in the traditional non-Hodgkin's lymphomas (NHL), such as poor performance status, multifocal disease, and increased age, are also poor prognostic factors in posttransplant lymphoproliferative disorders (PTLD). Rituximab has proven efficacy in CD-20 positive PTLD and has altered the management and prognosis of this disease.
