Pulmonary segmentectomy under total thoracoscopic procedure
2004年以後,当科で完全胸腔鏡下肺区域切除術の予定で手術を施行した28例について検討した.2例が開胸に移行したが,その内1例は腫瘍学的理由による開胸肺葉切除への予定的変更例である.それ以外の27例中,予定外開胸例は出血による小開胸への変更1例で,胸腔鏡下区域切除の完遂例は26例96%であった.出血量は5~305ml(中央値98ml),手術時間は147~313分(中央値222分)であった.完遂26例中17例(65%)で術後肺瘻を認めず,胸腔ドレーン留置期間は1~7日(中央値1日)であった.術後合併症は皮下気腫,気管支潰瘍が各1例で重篤なものを認めず,術死,院内死を認めなかった.完全胸腔鏡下肺区域切除術は,現時点では高い熟練度と時間を要する手術手技であるが ...
