Necrotizing enterocolitis in one of pygopagus conjoined twins developed 72 hr after delivery. The unaffected twin supported the postcolectomy twin through sepsis, renal failure, and hyperbilirubinemia, permitting planned surgical separation at 2 mo of age.

A case is presented wherein a set of black female pygopagus siamese twins were successfully separated by our department in their 45th day of life. Both twins had multiple malformations: cardiopulmonary, vertebral, anorectal, genitourinary, etc. The bilateral renal absence in one of them, originated her death on the 7th postoperative day.

This is a case study of malformed conjoined twins in a spontaneous triplet pregnancy. A 20-year-old primigravida spontaneously delivered at 22 weeks gestation a normal 425-gram male newborn followed by a delivery of a 525 grams pygopagus conjoined male twins.
