Double pylorus

Double pylorus is a rare abnormality and it occurs most commonly secondary to chronic peptic ulcer disease. It is usually treated conservatively with anti-ulcer therapy. However, refractory cases may require surgery.

The Double Pylorus

Two patients with double pylorus and two with prepyloric mucosal septum are reported. Double pylorus has previously been supposed to be congenital. Our findings suggest that it is an acquired lesion, caused by penetration of an ulcer through the pyloric ring.

In recent years, however, some case reports have indicated that this lesion may be acquired (Hansen Kronborg and Pedersen, 1972; Drapkin et al., 1974; Farack et al., 1974).

Acquired Double Pylorus

We have reported two cases of double pylorus or duplicated pyloric channel as an unusual complication of peptic ulcer disease. It is usually noted as an incidental finding on x-ray films or at endoscopy. Double pylorus seems to be an acquired lesion arising from gastric ulceration and does not necessarily require surgical correction.

Double channel pylorus

Radiographic and clinical features in five patients with the double channel pylorus deformity are reviewed and compared with previously reported cases. The double channel pylorus is a form of gastroduodenal fistula with a characteristic radiographic appearance.

The multi-channelled pylorus

The entity of multiple pyloric channels may be congenital but the majority are acquired lesions representing fistulae secondary to peptic ulcer disease in the pyloroduodenal region. The fistula usually joins the lesser curvature of the stomach with the superior aspect of the duodenal bulb.

Reconstruction of the pylorus

AbstractReconstruction of the pylorus has been performed on 12 patients who had previous vagotomy and pyloroplasty and whose symptoms of bile reflux and vomiting, dumping, or diarrhea had not been satisfactorily controlled medically. This technically simple operation was followed by marked improvement in 60% of patients.

Double Pylorus—In Evolution

Double pylorus is a rare abnormality. Over a 4-year period, we have found two cases of double pylorus in 2,500 endoscopic examinations, an incidence of 0.08%. In one patient, the development of double pylorus from a prepyloric ulcer was followed by serial endoscopies. Most cases of double pylorus are acquired from peptic ulcer disease.
