Pyruvate kinase electrophoresis in normal and pyruvate kinase deficient hemolysates

Abstract Two bands of pyruvate kinase activity were demonstrated in normal hemolysates with high voltage electrophoresis at pH 5.3 using cellulose acetate strips. Heterozygotes with red cell pyruvate kinase deficiency also exhibited two bands. Six of ten homozygous patients showed two bands, the remaining four only one band.

Pyruvate kinase deficiency is the second-most common hereditary nonspherocytic hemolytic anemia. Only four affected patients' pregnancies have been reported since 1980.A 22-year-old without prior splenectomy experienced severe hemolytic anemia, with hematocrit as low as 19.0%, requiring serial transfusions.

Hereditary nonspherocytic hemolytic anemia is a relatively recently recognized entity. It was first discussed in detail by Dacie et al, in 1953,1 and by 1960 over 60 cases had been reported.2 Motulsky et al.3 noted increased 2,3-DPG levels in several patients with hereditary nonspherocytic hemolytic anemia.

[Diagnosis of pyruvate kinase deficiency].

Two children with nonspherocytic anaemia and pyruvate kinase deficiency had their diagnosis delayed due to normal initial estimations of enzyme activity. Repeated analyses involving other glycolytic enzymes documented an increased glucose-6-phosphate dehydrogenase activity as a function of high reticulocyte counts.
