A 56-year-old man presented with typical angina for 1 day and a prior history of exertional dyspnea of 6 months duration. Clinically, he was diagnosed with severe aortic regurgitation (AR). Electrocardiogram showed left ventricular hypertrophy with volume overload.

Congenital quadricuspid aortic valve
The American Journal of Cardiology, 1966Abstract This case of a patient with a congenital quadricuspid aortic valve is the seventh to be reported in the world literature. Our report indicates that a quadricuspid aortic valve appears to function as well hemodynamically as a normal cuspid valve and therefore was discovered only as an incidental finding in this 67 year old man.

Congenital quadricuspid aortic valve
The Annals of Thoracic Surgery, 2001Our patient was a 53-year-old man referred for coronary revascularization and a history of a heart murmur. Intraoperative transesophageal echocardiography demonstrated a quadricuspid aortic valve (Fig 1) with opening of all four leaflets during systole (Fig 2). Severe aortic insufficiency was detected with color flow Doppler (Fig 3).

Normal aortic valve has three leaflets. Quadricuspid aortic valve(QAV) is a rare congenital anomaly of aortic valve in which there are four cusps. These patients should be diagnosed and clinically monitored as they are prone to developing aortic regurgitation in their life.
