In a breast cancer patient with acute hypercapnic respiratory failure, paraneoplastic ALS was diagnosed despite negative onconeural antibodies. Red flags included diaphragmatic paralysis, ESR of 120 mm/h, and CSF protein of 246 mg/dL. Clinicians should consider this diagnosis even when antibody panels are negative.
