In this post‐transplant patient, increased hyaline casts with negative dipstick protein but positive SSA testing occurred during evolving acute kidney injury and resolved with renal recovery. This discordant urinalysis pattern may warrant confirmatory protein testing and correlation with serial renal function in patients at high risk of AKI.

Point‐of‐care ultrasound performed at the bedside identified hepatic portal venous gas in a critically ill neonate, prompting urgent evaluation and revealing intestinal perforation due to necrotizing enterocolitis.
