ABSTRACT Despite the increasing prevalence of airborne infections such as COVID‐19, congenital hiatal hernia should be suspected in preterm infants presenting with respiratory symptoms and recurrent reflux. Barium swallow is helpful in ruling out pneumonia. Antenatal diagnosis is a key to early recognition and timely interventions.
