BMJ, 2013I did not suggest that anticoagulation is ineffective in atrial fibrillation1 2: its effectiveness is not in dispute. The issue is the relative benefit of anticoagulation. If the background rate of stroke declines as it has, the absolute benefit of anticoagulation will also decline.
