Model-Free Reinforcement Learning for Automated Fluid Administration in Critical Care [PDF]

open access: yesarXiv
Fluid administration, also called fluid resuscitation, is a medical treatment to restore the lost blood volume and optimize cardiac functions in critical care scenarios such as burn, hemorrhage, and septic shock. Automated fluid administration systems (AFAS), a potential means to improve the treatment, employ computational control algorithms to ...

A new framework for tailoring laparoscopic cholecystectomy: Integrating preoperative clinical factors with surgical difficulty based on the Tokyo Guidelines 2018

open access: yesJournal of Hepato-Biliary-Pancreatic Sciences, EarlyView.
Abstract Purpose The Tokyo Guidelines 2018 introduced the Surgical Difficulty Score (TGDS18) to assess laparoscopic cholecystectomy (LC) difficulty based on intraoperative findings. This study aimed to predict surgical difficulty preoperatively using clinical factors correlated with TGDS18. Methods Of 369 LC cases for cholecystitis (Jan 2014–Jul 2024),

Opportunities and challenges of mRNA technologies in development of Dengue Virus Vaccine [PDF]

open access: yesarXiv
Dengue virus (DENV) is a mosquito-borne virus with a significant human health concern. With 390 million infections annually and 96 million showing clinical symptoms, severe dengue can lead to life-threatening conditions like dengue hemorrhagic fever (DHF) and dengue shock syndrome (DSS).

Preemptive Propofol Administration in Spinal Cord Injury: Effects on Pain‐Induced Hypertension, Neuroinflammation, and Functional Recovery in Rats

open access: yesThe Kaohsiung Journal of Medical Sciences, EarlyView.
ABSTRACT Spinal cord injury (SCI) triggers secondary damage, including pain‐induced hypertension, inflammation, and hemorrhage, impairing recovery. This study evaluated the efficacy of general anesthesia with preemptive propofol administration in mitigating secondary damage in SCI rats. SCI was induced in rats using a contusion model.

Handling missing values in clinical machine learning: Insights from an expert study [PDF]

open access: yesarXiv
Inherently interpretable machine learning (IML) models offer valuable support for clinical decision-making but face challenges when features contain missing values. Traditional approaches, such as imputation or discarding incomplete records, are often impractical in scenarios where data is missing at test time.

Postoperative Bleeding, Revision Surgery, and Outcome of Cold Steel Tonsillotomy in Children With Upper Airway Obstruction

open access: yesThe Laryngoscope, EarlyView.
In this study, we present the results of a single‐center, 10‐year retrospective cohort study involving 1810 pediatric tonsil surgeries. The aim was to evaluate the safety and outcome of cold steel tonsillotomy with adenoidectomy. The analysis includes postoperative bleeding rates, rates of revision surgery, and caregiver‐reported symptom relief ...

Otopathologic Patterns of Cellular Degeneration in the Peripheral Vestibular Organ Secondary to Head Trauma

open access: yesThe Laryngoscope, EarlyView.
This study examines vestibular degeneration following head trauma, comparing cases with and without fractures. Histopathological analysis of 26 temporal bones showed more severe damage in nonfracture cases, particularly in Scarpa's ganglion cells and vestibular hair cells.

Blood autotransfusion outcomes compared with Ringer lactate infusion in dogs with hemorrhagic shock induced by controlled bleeding

open access: yesJournal of Research in Medical Sciences, 2011
Background: The most common cause of shock in the surgical or trauma patient is hemorrhage. Crystalloid solutions and blood transfusion are the mainstays of treatment of hemorrhagic shock.

Dual-Task Vision Transformer for Rapid and Accurate Intracerebral Hemorrhage CT Image Classification [PDF]

open access: yes
Intracerebral hemorrhage (ICH) is a severe and sudden medical condition caused by the rupture of blood vessels in the brain, leading to permanent damage to brain tissue and often resulting in functional disabilities or death in patients. Diagnosis and analysis of ICH typically rely on brain CT imaging.

Is Ophthalmology Consult Needed for Acute Orbital Fractures?

open access: yesThe Laryngoscope, EarlyView.
ABSTRACT Failure to recognize concomitant ocular injury in orbital fracture patients is a feared consequence of evaluation in the acute setting. These injuries and their accompanying functional sequelae range in severity, and reconstructive surgeons need to be sensitive to the possibility of major ocular injuries that could potentially have serious ...
