A Case Report of Papillary Adenocarcinoma of the Sigmoid Mesocolon.

腸間膜の充実性腫瘍はまれな疾患であり, ほとんどすべての悪性腫瘍は肉腫か中皮腫である.左下腹部有痛性腫瘤を主訴に来院した42歳女性のS状結腸間膜の乳頭状腺癌の1症例を経験した.腫瘍は手拳大の大きさで, S状結腸間膜内に限局し, 腫瘍とともに左半結腸切除術を施行した.病理組織検査では高分化乳頭状腺癌と未分化癌の混在を認め, 卵巣の表層上皮性腫瘍 (漿液性乳頭状腺癌+類内膜癌) に酷似していた.特殊染色や免疫組織化学的検討にて中皮腫や肉腫的性格は認められず, 婦人科臓器, 消化器臓器の綿密な検索を行ったが, 結腸間膜以外に原発を疑う臓器は認められなかった.欧米および本邦の文献には同様の報告はきわめて少ない.この腫瘍の組織発生について考察を加えた.術後3年の現在, 再発の兆候なく元気に日常生活を営んでいる.
