A 13-year-old girl presented at our hospital with a 3-month history of nasal obstruction, headache, hearing loss and severe weight loss. She also had nasal obstruction with occasional bleeding and a persistent ‘nasal twang’. These symptoms had previously been referred to as chronic sinusitis.

Upper respiratory tract infection and allergic inflammation are recognized as the important risk factors for acute sinusitis, with upper respiratory tract infection being most common. In children with acute or chronic sinusitis, the respiratory symptoms of nasal discharge, nasal congestion and cough are usually prominent.

An encephalocele is a protrusion of the cranial contents beyond the normal confines of the skull. It is a rare cause of seizure in adults. A 38-year-old woman presented with a first-onset seizure. Brain CT was interpreted as right frontal sinus opacification suggestive of sinusitis. The patient was discharged home with an amoxicillin prescription.
