Acta Neurochirurgica, 1993A series of 35 selected non senile patients was investigated with stereotactic brain biopsies in frontal and temporal lobes including the rhinencephalon. Results of neuropathological studies are confronted with clinical proposals. Alzheimer's disease is diagnosed in about 10% of the cases under 50 years of age, but more than 50% after 50.

There are few parts of the body which may not be approached by means of a surgical operation. An exception to this rule is the deep parts of the brain. The reasons are easy to understand. The head is a rigid cavity and about 90% by volume of its contents is cerebral tissue. This tissue tolerates retraction and displacement badly.

The first biopsy in Val-de-Grâce has been performed in 1987, under CT alone, with a Leksell frame. 150 punctures have been performed for a diagnostic or a therapeutic goal. Since 1990, stereotaxy is performed under MRI. 320 punctures have been made with 1.9% mortality and 1.3% morbidity.
