In the past decade, the definition of stroke has been revised and major advances have been made for its treatment and prevention. For acute ischaemic stroke, the addition of endovascular thrombectomy of proximal large artery occlusion to intravenous alteplase increases functional independence for a further fifth of patients.

Preventable stroke and stroke prevention
Journal of Thrombosis and Haemostasis, 2005Stroke is a major cause of death and disability in the world. The main causes of stroke are atherothromboembolism and cardiogenic embolism. The main causal and treatable risk factors for atherothromboembolic ischemic stroke are increasing blood pressure (BP), increasing cholesterol, cigarette smoking and diabetes; and the main risk factors for ...

Illicit drugs, appetite suppressants, decongestants, and anabolic steroids may cause stroke. Drug abuse is a common cause of stroke in young patients. Mechanisms are varied, but it is particularly important to seek out infective endocarditis and most importantly, cerebral aneurysms, vascular malformations, and cardiac disorders in cocaine abusers. The annual incidence of stroke in the elderly approaches 2 per cent. The major tasks of the physician are to attempt to prevent recurrence and to supervise rehabilitation. Prevention of recurrence involves a careful consideration of differential diagnosis, treatment of contributory disease, and the initiation of therapy specific to large vessel ...

When is a stroke not a stroke?
British Journal of Neuroscience Nursing, 2006There has been significant focus on stroke in the mass media since December 2005, following the stroke suffered by Ariel Sharon, the Israeli Prime Minister, and the death in January of Tony Banks as a result of a similar catastrophic vascular event. It has been interesting to notice the way in which these cases have been described in news broadcasts ...

During the past year epidemiological studies have linked elevated plasma total homocysteine concentrations with an increased risk of ischaemic stroke because of arterial disease. Laboratory studies have further explored the mitogenic effects of total homocysteine on vascular smooth muscle, and cytotoxic and thrombophilic effects on vascular endothelium.

There is evidence to support rehabilitation in well coordinated multidisciplinary stroke units or through provision of early supported provision of discharge teams. Brain infarcts and brain hemorrhages in patients younger than 20 years are significantly less common than among those older than 65 years; also, their clinical expressions are different from those common to older patients. Congenital defects involving the heart and the arteries supplying the brain are among the most common causes of stroke in the young.
