Subtrochanteric missile fractures of the femur

The treatment results of subtrochanteric missile fractures of the femur in 41 casualties are analyzed. The wounds were inflicted by infantry weapon missiles in 29 (70.7%) and by fragments of mine or explosive devices in 12 (29.3%) casualties. The associated neurovascular bundle injuries were present in 11 (26.8%) patients.

Subtrochanteric fractures are relatively rare in children and usually result from severe trauma. The unique biomechnical forces about the hip following fracture tend to complicate treatment. Remodeling and growth stimulation generally ensure good results by nonoperative means in children under 10 years of age.

The results of treatment of fifty-six patients with subtrochanteric fractures of the femur are presented and a new classification of these fractures is described. In forty-seven fractures treated with internal fixation there were nine failures of fixation.

Subtrochanteric fractures of the femur comprise only 5% to 7% of hip fractures, but are important because of the difficulty in management. They combine the problem of instability to varus deformation common to comminuted intertrochanteric fractures and the problem of delayed union common to diaphysial fractures of the femur.
