Annals of Gastroenterological Surgery, EarlyView.This multicenter retrospective study analyzes real‐world management of acute cholecystitis in patients aged ≥ 70 years across five institutions, focusing on adherence and intentional deviation from TG18 recommendations. By classifying treatment strategies as TG18‐accordant, more aggressive, or more conservative, we reappraise current practice patterns.
