Superior Hypogastric Plexus Nerves

Abstract The superior hypogastric plexus block (SHP) is performed for pain originating from pelvic organs, mostly from malignant sources. It can also be performed in patients in whom treatment is completed but radiation, scar tissue, or other treatment-related side effects have resulted in pain in the pelvis.

The preaortic sympathetic plexus is a continuation of the thoracolumbar sympathetic chain extending down anterior to the aorta and vertebral bodies in the retroperitoneal space (Fig. 36.1). The inferior hypogastric plexus is at approximately the L3–L4 level and the superior hypogastric plexus at L4–S1.
