Surgical Devices for Creating Stomas.
Nippon Daicho Komonbyo Gakkai Zasshi, 1999 セルフケアが容易で, 合併症のないストーマを造設することは患者のQOL上極めて重要である.ストーマの形態としては, 高齢者のオストメイトの増加を考慮し, 洗腸療法, 自然排便法の双方に対応できる突出型のストーマを造設しておくのが良い.それでも皮膚切開の大きさや, 挙上腸管の長さ, 縫合法などにより術後のストーマの形状はさまざまである.著者らは過去7年間, 20例の単孔式結腸ストーマ造設に際し, 皮膚切開の大きさ, 挙上腸管の長さを測定し, その結果としてのストーマの最大径, 高さを計測してきた.種々の検討の結果, 皮膚切開は2.5×1.5cm (縦×横), 挙上腸管の長さを3.0～3.5cmとすると, ストーマの最大径は2.5cm, 高さ1cm程度の円形のストーマが造設され満足しうる結果を得た.
