Tandem Occlusion Causing Acute Ischemic Stroke
2019Tandem occlusion (TO) is defined as the presence of concurrent large intracranial and extracranial artery occlusion causing an acute ischemic stroke (AIS). The stroke mechanism is usually an artery-to-artery embolization of a thrombus formed at the site of cervical carotid obstruction, which may be caused by atherosclerotic disease or dissection.

The management of cervical artery occlusions in hyperacute stroke with tandem cervical/intracranial occlusions has not yet become standardized, especially when the circle of Willis is effective. We retrospectively analyzed the safety and accuracy of current approaches to manage the cervical occlusion in consecutive patients with tandem occlusions ...

Carotid artery angioplasty versus stenting for management of acute tandem occlusions
Journal of the Neurological Sciences, 2021Background The optimal approach to treat the cervical carotid artery lesion during endovascular thrombectomy (EVT) for acute strokes with tandem occlusions is unclear. While carotid artery stenting (CAS) might be a more definitive recanalization method, the potential risk of intracranial hemorrhage (ICH) limits its routine usage.

Endovascular treatment of tandem vascular occlusions in acute ischemic stroke
Journal of NeuroInterventional Surgery, 2014Background and purposeTandem vascular occlusions are an important cause of acute ischemic stroke (AIS) and present unique treatment challenges. We report our experience of managing a subset of AIS patients with extracranial vascular stenting/angioplasty and intracranial revascularization.MethodsConsecutive patients who presented at three centers with ...

Tandem Occlusions (Antegrade First Technique with Stent)
2019A 70-year-old male was admitted with 5 h duration of left-sided weakness. The NIHSS score was 17, and the patient had a tandem occlusion; there was a tight stenosis at the origin of the right ICA along with M1 MCA occlusion. There were good collaterals along with a small infarct core (Fig. 90.1).

Mechanical thrombectomy in tandem occlusion: procedural considerations and clinical results
Neuroradiology, 2014Acute tandem occlusions of the cervical and distal internal carotid artery (ICA) or middle cerebral artery (MCA) are associated with major stroke with intravenous (i.v.) thrombolysis alone in approximately 90 % of patients. The data on endovascular management of tandem occlusions is still limited.

[Tandem occlusions in acute ischemic stroke].
Der Radiologe, 2019Acute strokes caused by tandem occlusions include an atherosclerotic or dissective stenosis/occlusion of the extracranial internal carotid artery (eICA) in combination with an intracranial vessel occlusion.Endovascular treatment can be technically challenging but is definitely superior to intravenous thrombolysis alone and achieves good clinical ...

Intracranial Occlusion with Tandem Carotid Stenosis: Retained Filter
2019A 70-year-old diabetic and hypertensive presented at 2 h with right-sided weakness and speech disturbance. Examination revealed dense right-sided weakness and global aphasia adding up to a NIHSS score of 19. Tandem left ICA and M1 occlusion was noted on CTA.

E-158 Tandem carotid occlusions with delayed carotid stenting
Electronic Poster Abstracts, 2018Introduction/purpose Management of tandem carotid occlusions is not yet well-defined. Though several authors have proposed acute carotid stenting followed and intracranial mechanical thrombectomy, the subsequent need for immediate antiplatelet therapy comes with an associated risk of potentially devastating hemorrhage.
