To the Editor:— In your question and answer section ( 198 :493, 1966) there appears a query about reactions to tetanus toxoid. My answer to the question would differ completely from that of Dr. Dull. In my experience, local allergic reactions to tetanus toxoid are fairly common and generalized reactions are not rare.

To the Editor.— I would like to report a patient who received an overdose of "tetanus toxoid, fluid." A total dose of 3.8 ml (2.0 ml intramuscularly in one buttock, 1.6 ml in the other, and 0.2 ml subcutaneously in the left deltoid region) was inadvertently administered to a 32-year-old female employee.

Allergic responses beyond mild, transient injection-site erythema and induration are fortunately rare. A rather severe systemic reaction and a unique local reaction are reported here.
