Abstract Medial open‐wedge high tibial osteotomy is an established treatment for medial compartment osteoarthritis in the varus knee. The conventional ascending tibial tubercle technique can lower patellar height and increase the tibial tuberosity‐trochlear groove distance, raising concerns for secondary patellofemoral disorders.
